How do i ask this girl out?

ok well im 12 just started 6th grade.im kinda shy tall skinny/my frinds call me mr stick and i get to be woth my old freinds that moved away wich doesent have much to do with this.ok i found out this girl likes me her name is lindsey last week from my dumbsrry freind.she didnt want me to no tho since shes real shy to.anyway wen i found out i didnt no wich lindsey is her so i asked my freind and he pointed tourd her and then during lunch break he came by with a note that he wrote o ya and their was this dance at my school 2 days later anyway he wrote it and it asked her if she wanted to goo to the dance with me.i tried to get it from him but then he ran off right before the bell rang.he gave it to her and i didnt no he did becouse i thot he was joking wen he did it.i went to the dance becouse i wanted to ask her their and be with my freinds.wen her freind came up to me she asked me to go dance with her and wen i came up to her she said i read that note u gave me and i thot uhh wat and then i said that note was fake then she got all sad and i didnt no watd happened.her freinds got mad at me and i didnt no y.i went and asked my freind and he said he did give it to her.now its this week and ive been to scrared to tell her anything since she mighht be mad.today i was able to tell her freind becouse she was in some of my cles.i asked her if she could explain it to her she said yes so at the end of school i saw they wer talking and im guessing she nos the truth then.so tomorrow i was gnna ask her out but i always get real scared at things like this.should i give her a note that i wrote and is serious about or should i ask her myself or wat i dont no wat to do.can u plz help me?sorry about realy loooooooooooooooooong guestion
